Show / Hide Fields Using Conditional Logic

You may want certain fields to show or hide based on the client’s answer to another question i.e. If the client selects ‘Yes’ for the question ‘Do you have any allergies?’ a text area ‘Please specify:’ will show. If the client selects ‘No’ the text area ‘Please specify:’ will be hidden from the form.

From the Form Builder click on ‘Edit’ next to the field you want to set the conditions for / Field Conditions / Initial State: Hide / Add Condition / Add Logic / Field: Select the field you want these conditions to be based on (We recommend selecting a picklist field) / Operator: Equal to / Value: Yes / Result State: Show / Save.

This means that the field will be hidden from the form unless the client selects ‘Yes’ in the chosen field.
